- Personal Social Security Number
- Home address history for the past three years
- Your current driver license number, along with your driver license history for the past three years
- Employment history for up to 10 years
- A complete history of traffic accidents, violations, and/or convictions from the last three years (This includes any DUI or reckless driving conviction and license suspension)
